Technically, Day 5 embryo is more superior compared to Day 3 embryo as a Day 5 embryo (blastocyst) contains more cells and enables an embryologist to evaluate and select the best quality embryo that will most likely increases the chance of pregnancy. Hence, avoiding transferring more than 1 embryo per transfer. However, not all embryos will develop into blastocysts, hence depending on your fertilisation, you should always work closely with your doctor to optimise your chance and avoiding the risk of having no blastocyst transfer on Day 5 or Day 6.

This should be individualized and no blanket rule should be applied to all couples in order to decide which is the right option whether a couple should have a day 3 or 5 embryo transfer. The decision will need to be based on several factors. The available number and quality of embryos, age of the woman, the number of embryos that the couple wish to transfer, their preferences and previous history of fertility treatment outcome. Several studies have been done to compare between the two and after taking into consideration all factors, there seems to be no advantage to transferring on day 5 vs day 3. However, it may be considered in certain cases whereby additional embryo selection is necessary for e.g the patient has a lot of good embryos and is keen to do a single embryo transfer following a PGT-A.
